How does the scores/goals feature work?

 Introduction

This feature lets practitioners track client progress over time by defining goals. Colib supports two types of goals:

- Scale of options — the client (or practitioner) picks an answer from a predefined scale, where each option combines a text label with a numeric value (e.g. Never / Sometimes / Often).
- Numeric measurement — a free numeric value with a unit, ideal for tracking health measurements such as weight (kg or lb), blood pressure (mmHg) or blood glucose (mmol/L).

Goals are organized into categories (e.g. Anxiety assessment, Health measurements), and each category is displayed as its own table in the client file, with progress charts.

Feature Configuration

Goals now have their own settings tab: go to Settings > Goals (https://www.colib.io/settings?show=goals) and enable the feature with the toggle.







Categories.

 Click '+ Add a category' to create your first category. You can rename a category at any time; a category can only be deleted once it no longer contains any goals. If you were already using Scores/Goals before this update, all your existing goals were automatically placed in a category named "Default" — you can rename it or create new categories and reorganize your goals.

Goals. 

Inside a category, click '+ Add a Score/Goal'. In the dialog:

1. Enter the name and the description (the description is what your clients see on forms and on the chart).
2. Select the category the goal belongs to.
3. Choose the type:
- Scale of options: define the number of options and the text of each one (optionally starting at 0).
- Numeric measurement: enter the unit displayed next to the values (e.g. kg, lb, mmHg, mmol/L). No options are needed.
4. Save.

Note: once a goal has recorded entries, its type can no longer be changed (scale values and measurements are not
convertible). You can still edit its name, description, category or unit.

Tip: for blood pressure, create two measurement goals (Systolic and Diastolic) in the same category.

Integrating Goals Into Forms

You can ask clients to record their answers and measurements directly through your forms:

1. Create a new section within the form builder
2. Select 'Score/Goal' as the section type
3. Choose a category — the list below then shows the goals of that category
4. Select the goals to include and click 'Save'

On the form, scale goals appear as a grid of options, and measurement goals appear as numeric fields with their unit (both are required). Submitted values are saved to the client's record automatically and appear on the completed form's PDF.







Impact on Client Records

Once activated, the client record displays a Scores/Goals section with one table per category. Each column is a date:

- Scale goals: pick an option from the dropdown for today ('Today' column) or edit a past entry. The Sum and Average rows at the bottom of the table are calculated on scale goals only.
- Measurement goals: type the value directly in the numeric field (the unit is shown next to the goal's name). You can correct a past value at any time; clearing the field deletes that day's measurement. If several values are recorded on the same day (e.g. through multiple form submissions), the most recent one is displayed.

Clicking the analytics icon beside each goal displays a chart tracking that goal's evolution over time — for measurements, the chart uses the exact decimal values.
