If you work with children, you might need to collect information about both the child and their guardian when sending an intake form to your clients. Colib has you covered for this.
When you create a new question of type "Text (single row)", you can map the question to a client field. The following four fields are relevant in this case:
- Minor/Client - First Name
- Minor/Client - Last Name
- Parent/Guardian - First Name
- Parent/Guardian - Last Name
How the platform determines who fills out the form
Form intended for the parent/guardian
- If at least one question is mapped to "Minor/Client - First Name" or "Minor/Client - Last Name", the platform assumes the parent/guardian is filling out the form.
- After submission, two client profiles will be created:
- A Parent/Guardian profile with all the provided information.
- A Minor/Client profile containing only the first and last name from the mapped fields.
Form intended for the minor
- If at least one question is mapped to "Parent/Guardian - First Name" or "Parent/Guardian - Last Name", the platform assumes the minor is filling out the form.
- After submission, two client profiles will be created:
- A Minor/Client profile with all the provided information.
- A Parent/Guardian profile containing only the first and last name from the mapped fields.
When fields for both minor and parent/guardian are present
- If the form includes fields mapped to both the minor and parent/guardian, the platform ignores the fields related to the parent/guardian.
- Contact information (phone and email) will be shared between the minor and the parent files.
After the two client files are created, they will be automatically linked via a Family Group.
