Who is this article for?
Your intake form asks for the client's address, but the address does not appear in the client record after the form is submitted. For Colib to save it, the form needs the right questions, linked to the right client fields, with the right type of question. This article lists them and explains how to set them up.
Before you start: the link between forms and client records must be on: in Settings > Forms (www.colib.io/settings?show=signature), Link every completed form to a client record must be turned on. Without it, the Client Mapping list does not appear in your questions. To learn how mapping works in general, please see How to connect your client fields to your form questions.
1The questions your form needs
| Question in the form | Type of question | Client Mapping | Needed? |
|---|---|---|---|
| Street address | Text (single row) | Street address | Recommended |
| City | Text (single row) | City | Required |
| Province | List of values | Provinces | Required |
| Postal code | Text (single row) | Postal/Zip Code | Recommended |
The province must be a List of values question: Colib needs to know the province exactly to save the address, so Provinces is only offered for a List of values question. An older form may still have a Text question mapped to Provinces: that answer is ignored, and the address of a new client is then not saved at all. This is the most frequent cause of a missing address.
2Add the questions
For each line of the table above:
- Go to Forms (www.colib.io/forms), click the pencil icon of the form, then Sections & Questions.
- Click Add Question in a section (or the pencil icon of the existing question).
- Type the question, for example Province, and choose its Type.
- In Client Mapping, choose the matching client field.
- Turn on Mandatory for the city and the province, so the client cannot skip them.
- Click Save. A mapped question shows a gears icon in the list of questions.
For the province, you do not type the choices: when List of values is mapped to Provinces, Colib fills in the list of the Canadian provinces and territories itself, and its choices cannot be changed.
Reminder in the editor: when you map a question to Street address, Colib reminds you that the form must also contain questions mapped to the city and to the province of the client.
3What happens when the form is submitted
- New client — the record is created with the address entered in the form.
- Existing client — the address of the record is updated with the answers of the form. A part of the address that the form does not ask for, or that the client leaves empty, keeps the value of the record (the city too, as long as the province does not change).
- City — the city is matched with the cities of the chosen province. A city that Colib does not know yet is added.
- Country — addresses saved from a form are Canadian addresses.
Ask for the whole address: map the four questions of the table and make the city mandatory. For a new client, Colib can only save the parts of the address that the form asks for.
The address is still not saved?
| Check | What to do |
|---|---|
| The province question is a Text question (older form) | Change its type to List of values and map it again to Provinces. |
| There is no question mapped to the city or to the province | Add the missing questions (step 2). |
| Link every completed form to a client record is off | Turn it on in Settings > Forms. |
| The form was submitted before the mapping was saved | Mappings only apply to the forms submitted after them. Enter the address by hand in the client record, or have the form filled out again. |
| The question is shown only on condition and stayed hidden | A hidden question is not copied into the record. Check the condition of the question. |
